Routine immunizations and new vaccine introductions are two best buys in global health. While immunization coverage has increased dramatically in the last decade, more must be done to ensure every child is reached. To bridge this gap, we need reliable, easily accessed and actionable data on the barriers impeding immunizations, coupled with trained data users at all levels of the health system. The BID Initiative is grounded in the belief that better data, plus better decisions will lead to better health outcomes. Partnering with Tanzania and Zambia, the BID Initiative developed, tested, and rolled out interventions that address some of the most pressing routine immunization service delivery challenges, such as poor visibility into vaccine supplies and difficulty identifying children who default on immunization schedules. These seven briefs summarize the BID Initiative’s approaches and interventions, and shares recommendations and lessons learned for others interested in improving immunization data quality and use.

Digital immunization registry: Evidence for the impact of mHealth on enhancing the immunization system and improving immunization coverage for children under one year old in Vietnam

The Vietnam National Expanded Program on Immunization (NEPI) has been successfully implementing a nationwide immunization system since 1985. From the start, the program has increased the immunization coverage rate; however, data on immunization coverage in Vietnam are gathered and aggregated from commune health centers in routine, paper-based reports, which have shortcomings. Also, calculations of coverage are inconsistent at subnational levels, which lead to uncertainty about the size of the target population used as the denominator in coverage calculations. The growth of mobile networks in Vietnam provides an opportunity to apply mHealth to improve the immunization program. In 2012, PATH and the Vietnam NEPI developed and piloted a digital immunization registry, ImmReg, to overcome the challenges of the paper system. A final evaluation was conducted in 2015 to assess the impact of ImmReg, including its use of SMS reminders, on improving the immunization program. Read the full article in the mHealth Journal here.

BID Initiative Theory of Change Interventions | Primary Outcomes 1 & 2

A theory of change (TOC) defines the activities, outputs, outcomes, evidence and assumptions on the pathway to achieve a long-term goal. For the BID Initiative, this pathway is based on the principal hypothesis that better information plus better decisions will lead to the long-term goal of better health outcomes. The TOC describes in detail the types of activities the BID Initiative is conducting to bring about the outputs and outcomes depicted in the pathway. The BID Initiative has four primary outcomes:
  1. Improved overall immunization data quality at scale in two demonstration countries by 2017
  2. Increased use of immunization data for decision making across all levels of the health system at scale in two demonstration countries by 2018.
  3. Achieved implementation of relevant components of the BID solution at scale in two demonstration countries, and commitment toward implementation by 5-8 other country governments within Sub-Saharan Africa by 2018.
  4. Significant additional resources are committed from donors, multilateral agencies, implementation organizations, or other innovative sources for financial and technical support to countries adopting and improving the BID solution by 2018.
There are a broad range of activities supporting the achievement of these outcomes, with the activities for primary outcomes 1 and 2 focused at the country level in Tanzania and Zambia (our two initial demonstration countries) and the activities for primary outcomes 3 and 4 focused mainly at the regional and global levels to expand and scale with additional resources. For this reason, we developed two separate but related TOCs – one for outcomes 1 and 2 and one for outcomes 3 and 4. In addition, there is a supporting document that summarizes a thorough review of the literature supporting the design of the BID Initiative activities and TOC related to outcomes 3 and 4.
